A Hydatic Cyst into a Large Diaphragmatic Hernia

A 44-year-old woman, presented to the emergency department with dyspnea since 1 month, associated with vomiting and right upper quadrant pain. She has a surgical history of hydatid cyst of the right liver for which a resection of the protruding dome was performed 18 years ago. On physical examination, she was tachypneic and pulmonary auscultation revealed bowel sounds in the right hemithorax with reduction of vesicular murmur. Chest X-ray showed bowel gas with air-fluid levelsinside the right chest cavity. A chest-abdominal computed tomography revealed a 9cm right diaphragmatic hernia which allowed passage of the right colon and gastric antrum into the chest cavity. The right lung was collapsed and the mediastinal shift towards the left. CT scan also revealed a 94*50mm multivesicular hydatid cyst strongly adherent to the liver and which is also included in the hernial content (figure). The patient was operated on. A resection of the hydatid cyst was performed, with reduction of the herniated organs into peritoneal cavity and diaphragmatic suture. The post-operative course was uneventful. Iatrogenic right diaphragmatic hernia is a rare complication after hepatic surgery. It can sometimes be difficult to identify at an early stage and can result in diagnostic delays with life-threatening outcomes. In our case , diaphragmatic injury and liberation of the liver during the first operation could explain hepatic location in the left abdomen and the occurrence of diaphragmatic hernia.

Bilateral Pyeloureteral Junction Syndrome on Horseshoe Kidney - A Case Report

Horseshoe kidney is the most common type of congenital renal fusion defect in children. The horseshoe kidney occurs in approximately 0.25% of the general population. Isolated horseshoe kidneys, which account for one-third of horseshoe kidneys, may be asymptomatic and go undiagnosed until adulthood. However, the presence of a horseshoe kidney in the pediatric population warrants further evaluation because the horseshoe kidney is frequently associated with other abnormalities: 78.9% including pyelourethral junction syndrome. In addition, the horseshoe kidney is associated with an increased rate of renal complications in adults, such as stones, ureteropelvic junction (UPJ) obstruction, trauma, infection, and tumors. Ureteropelvic junction syndrome is a disease that causes a failure of urine flow from the pelvis to the ureter. It is the most common cause of hydronephrosis in children. The diagnosis was made on the basis of symptoms, mainly pain and urinary tract infection, and required surgical treatment.

Human T-Cell Lymphotropic Virus-1 Causing Diseases and Cancer in Human

Human T-cell leukaemia virus-1(HTLV-1) is a kind of retrovirus which after entering in the human body reversely transcribed into several proviruses integrating the host genome develops a lethal blood cancer known as adult T-cell leukaemia (ATL). HTLV-1 also develops some other life threatening diseases like HTLV-1 associated myelopathy or tropical spastic myelopathy (HAM/TSP) in human. The present paper is an attempt to discuss discovery, epidemiology, diagnosis and transmission of the virus with their diseases and cancer causing abilities in the light of recent researches done so far in the field of microbial origin of cancer.

Burns can be caused by flames, ultraviolet radiation, hot liquids, electricity, lightning and certain chemicals. All burns require immediate first aid treatment. Partial and full thickness burns require urgent medical attention. Full thickness burns often require skin graft surgery. Simple analgesics (such as ibuprofen and acetaminophen) and opioids such as morphine didn’t used in 3rd degree burn because the burnt place is painless. The "Wallace Rule of Nines" is the most common method of determining body surface area. The most commonly used resuscitation formula is the Parkland formula which calculates the amount of fluid required in the first 24 hours.

Pyeloureteral duplication is a duplication of the excretory system of a renal unit. Renal duplication is a congenital anomaly, estimated in the general population at 1/125. Familial forms have been described and estimated at 1 in 8 in the siblings. Upper urinary tract malformations are not uncommon in adults and can lead to destruction of the renal parenchyma if diagnosis and management are delayed. Recurrent back pain is the most common symptom. It may also be due to infectious complications of the upper pyelo in stasis or due to ectopy. Urinary lithiasis is found in 26% of cases according to some series. Endoscopic incision in the context of ureteral duplicity, completed by ureteroscopy with drainage of the Upper excretory tract, may be the treatment of choice in adults. Simple, minimally invasive and with low morbidity, it allows an efficient drainage.

Studying the needs of patients with schizophrenia who are in a psychiatric hospital for a long time, while suffering from depression

Background. The purpose of this study was to study the needs of patients with schizophrenia who are in a psychiatric hospital for a long time, while suffering from paranoid schizophrenia and depression. It is known that depressive symptoms in patients with schizophrenia are associated with severe morbidity and mortality. As you know, schizophrenia is characterized by a chronic, incessant, severe course of the disease.


Method. The Calgary Depression Scale for Schizophrenia (CDSS) and DSM-5 were applied to 100 male and 100 female patients with paranoid schizophrenia who were found to have depression of varying severity. All patients were examined in the Republican Psychiatric Hospital No. 1 of the Ministry of Health of Azerbaijan, which is designed for 2000 beds Depending on the mental state of the patients; permission for the examination was obtained from the majority of patients or their guardians.


 Results. For the first time, the study found that men with paranoid schizophrenia who were in a psychiatric hospital for a long time, both in frequency and severity, were found to have depression of varying degrees. Men are twice as likely to be diagnosed with depression as women. This data obtained by us contradicts the occurrence of depression in the general population, where depression is usually twice as high in women. The data obtained were summed up both in terms of the severity of the clinical severity of depression DSM-5   and on the CDSS scale.


Conclusion. The results of this study suggest that men with paranoid schizophrenia who are long-term in a psychiatric hospital often suffer from depression than women. It is currently very difficult to fully explain this phenomenon. However, given that the husband in society occupies a dominant role as the head of the family, his long stay in a psychiatric hospital is one of the causes of depression. Thus, men with paranoid schizophrenia who are in a psychiatric hospital for a long time need more differentiated psychiatric help, especially in relation to affective disorders of the depressive type. This issue needs further study.
